- 18.5–24.9, healthy weight
- 25–29.9, overweight
- 30–34.9 (obesity (Class I)) may qualify for surgery with serious metabolic disease
- 35–39.9 (obesity (Class II)) likely qualifies with an obesity-related condition
- 40+ (severe obesity (Class III)) likely qualifies regardless of other conditions

Current ASMBS and IFSO guidelines recommend surgery at a BMI of 35 or higher regardless of other conditions, and consider it at a BMI of 30 to 34.9 with metabolic disease such as type 2 diabetes. These 2022 guidelines replaced the 1991 NIH criteria. Many commercial plans continue to use older coverage thresholds such as a BMI of 40 or higher, or 35 or higher with an obesity-related condition. Traditional Medicare is different: it requires a BMI of 35 or higher, at least one obesity-related condition, and documented prior unsuccessful medical treatment for obesity. Clinical eligibility and insurance coverage are not the same question, and we check both. Newer guidelines also recognize a BMI of 30 or higher with serious metabolic disease. This calculator estimates your BMI; only a surgeon can confirm candidacy (source: ASMBS/IFSO 2022 guidelines).
BMI is a useful screening number based on height and weight, but it doesn't measure body composition or distribution of fat. It's a starting point, your candidacy for surgery is determined by a full evaluation, not BMI alone.
