Coverage isn't as simple as a yes/no list. The same insurer can cover a procedure on one plan and exclude it on another, and medical-necessity rules differ by service line. Publishing a static payer list would mislead more patients than it helps. Instead, our team verifies your actual benefits (deductible, co-insurance, pre-authorization requirements) before anything moves forward.
When you meet documented medical criteria, most commercial plans and Medicare cover bariatric surgery and medically necessary GERD/hiatal hernia surgery. Our team handles pre-authorizations and letters of medical necessity on your behalf. We'll tell you exactly what your plan requires before you commit.
When venous reflux is documented by ultrasound and you have medically significant symptoms (pain, swelling, skin changes), treatment is typically covered by most major plans and Medicare. Cosmetic-only spider vein treatment is generally not covered. We verify and pre-authorize before your procedure.
